There is an important clarification on recently revised CGHS rates that central government employees, pensioners and ECHS beneficiaries should be aware of. The Ministry of Defence, through an addendum issued on December 16, 2025, has clarified how city-class and tier mapping will work for applying the new CGHS rates to ECHS-empanelled hospitals.
This clarification mainly affects how treatment rates are calculated in certain cities, especially those linked to HRA (House Rent Allowance) continuation rules.
Here’s a simple breakdown of what has changed — and what remains the same.
What is this addendum about?
Last month, the government revised CGHS treatment rates and extended them to ECHS-empanelled healthcare organisations (HCOs). However, there was confusion over which city category (X, Y or Z / Tier-1, Tier-2, Tier-3) should apply to some locations.
The December 16 addendum clears this confusion by clearly mapping certain cities to specific CGHS city classes, in line with HRA continuation orders.
In simple terms:
Which city category you fall under will decide which CGHS rate applies at an empanelled hospital.
Key city reclassification you should know
As per the latest clarification:
Treated at par with Delhi (X-Class / Tier-1)
These cities will now get CGHS rates applicable to Delhi:
Faridabad (Haryana)
Ghaziabad (Uttar Pradesh)
Noida (Uttar Pradesh)
Gurugram (Haryana)
This is significant because Delhi has the highest CGHS rates, which generally means higher reimbursement ceilings.
Jalandhar Cantt
Jalandhar Cantt will be treated at par with Jalandhar city
Applicable category: Y-Class / Tier-2
Goa and Port Blair
Both Goa and Port Blair (Andaman & Nicobar Islands) will be treated as:
Y-Class / Tier-2 cities for CGHS rate purposes
Chandigarh-linked cities
The following locations will be treated at par with Chandigarh:
Panchkula (Haryana)
S.A.S Nagar / Mohali (Punjab)
Applicable category: Y-Class / Tier-2
What remains unchanged?
Apart from the city re-mapping listed above, all other provisions of the CGHS rate revision issued earlier remain unchanged.
The clarification simply aligns CGHS rates with existing HRA continuation rules, as laid down by the Ministry of Finance.
The revised CGHS rates notified in October 2025 continue to apply — only the city classification interpretation has been clarified.
Why this matters for employees and pensioners
This update is important because:
CGHS reimbursement rates differ sharply between X, Y and Z class cities
Even a change in city classification can impact out-of-pocket expenses
It brings clarity for ECHS beneficiaries, especially in NCR and adjoining cities where confusion was high
Earlier, Financial Express had reported that the CGHS rate revision after several years was meant to better reflect rising healthcare costs. This addendum ensures that employees in satellite cities are not disadvantaged due to technical city classification issues.
Summing up…
No fresh rate hike has been announced but clarity on city-wise applicability will help smoother billing and reimbursement.
Employees and pensioners in NCR, Chandigarh region, Goa and Port Blair should check the updated city mapping before hospitalisation
For most beneficiaries, this clarification removes ambiguity — and ensures CGHS rates are applied fairly and uniformly across linked urban areas.
