The entry can’t be edited once payment is done

The entry form is available online and must only be submitted online with all the relevant details that are being asked for. The organisers will not be responsible for any incorrect or incomplete information. Any discrepancy arising out of such information will be the liability of the entrant and may lead to the cancellation of the entry.

Entries will not be considered complete until full payment has been made. Entries will be sent for judging only after receiving the payment.

Please don’t upload any supporting document, which is password protected.

An Audio-Visual or a Case Film or a PowerPoint Presentation about your entry is desirable but not essential. The Audio Visual/Case must not be more than 120 seconds and the PowerPoint Presentation must not have more than 10 slides. Specification for the video .MOV/ MP4, 350MB.

Any supporting document/Video/PowerPoint Presentation to be uploaded through Google Drive Link in a field mentioned in the online form. Please don’t upload any supporting document, which is password-protected, and make the drive open to all.

There is no limit to the number of entries an organisation, a hospital or an individual can submit. However, an all-inclusive, non-refundable entry fee should be paid online through a debit or credit card before final submission of the entry.

In case of any clarification required by the eminent jury or knowledge partner, the FE team may contact the nominee hospital/clinic/individual at any time during the voting process. It is the responsibility of the nominee to provide required details within the timeline suggested by the award organisers, failing which will lead to the drop of nomination.

- For National Level - Hospitals having presence in at least 2 regions/zones with minimum 5 branches across regions/zones.

- For Regional Level - Hospitals having presence in at least 2 states or cities within the regions/zones (as defined) with minimum 3 branches across states or cities.
